- Bring a photo ID and your Health Insurance card(s).

          $110 (High Dose - 65+) *

          $207 (COVID-19) *

* We can bill Medicare Part B & Medicare Advantage Plans, Michigan Medicaid * Medicaid Health Plans as well as some Private Insurance carriers. For HMO, contact your primary provider for options.
